I desire to apply for membership of Woodlands Tennis Club, to pay the proportionate subscription and to abide by the Rules and Regulations of the Club. Please indicate here that you have read and are in agreement with the above Cheques should be made payable to 'Woodlands Tennis Club' and sent to: Miss Louise Reader, 41 Ypres Road, Allestree, Derby All subscriptions are due annually from the date after they have been approved by the Annual General Meeting. Members likely to play in League Teams have to pay by 30^th April and no member should pay later than 31^st May. New members should pay as soon as their membership has been accepted. A written resignation is to be forwarded to the Membership Secretary when you cease to be a member of the club.
